Insurance Claim Service Center
Theptarin customer service team is ready to assist you in processing insurance claims.

For us to best serve you, please bring with you

1. Insurance cards
2. Photo identification card issued by a governmental agency

To claim for out-patient services

1.Show your insurance and identification cards to our receptionist.
2. After receiving medical service, the out-patient cashier will prepare for you the claim form and receipt. You then submit the two documents to your insurance company for claim.
3. In case that your insurance company has a contract with the hospital, you will need not to advance for charges which are covered in your policy. For charges that are not covered or the reason for sickness is unclear, you will be asked to advance the payment for those items.

To claim for in-patient services

1.Show our insurance and identification card to admission staff.
2. Customer service staff will visit you and explain details of your insurance coverage and how to process the claim during your stay.

To fax claim

1.Hospital staff will ask you to sign an insurance fax claim form to be submitted to your insurance company for approval, then will inform you of the reply from the insurance company.
2.If your admission claim is approved, please contact the In-Patient Cashier Department on 2nd floor, Building 1 prior to your discharge to sign insurance documents and make payment for any item not included in the approval.


In case you wish to process the insurance claim yourself or you cannot do fax claim, hospital will prepare for you on discharge day the necessary claim form, the receipt, and any other documents.

In case you need to submit claim to your employer

1. Please inform your wish to the admission staff.
2. On discharge day, in-patient cashier will prepare for you the medical certificate, copy of the receipt, and any other documents required by your employer.
Preparation for pre-insurance health screening
1. Have enough rest the night before.
2. Refrain from eating, drinking, and medication at least 8 hours prior to coming to hospital.
3. For urine collection, it is best to collect the mid-stream.
4. If suspect of pregnancy, please inform hospital staff immediately to skip x-ray.
5. For women, it is best to collect the urine 7-10 days after last day of menstruation.
